If you are talking about the demonology outfit, this one is implemented a bit awkwardly. You need to do the ending three and then you get a gallery picture with her wearing that outfit.
The action buttons are on the left side of the screen. If you have enough games to start an attack or if there is any other option there will be a button on the left side to press.
Attack you have to designate from which field you are attacking, which other field and so on
No there are only two undergroud acessible floors. The third one wrote that you do not have the key. It is not game breaking bug, but it doesnt look good. I started over on each difficulty and ACT and each time only two acessible underground floors.
There are three floors and the third one is required to progress in the game. (the level with the spectral slime) If you can send me a save game I'll analyze it.
If you are a patron you can post it on the discord bug channel. If not, just put it up on some cloud drive (mega etc.) and send me the link here and I'll have a look
I think the milking machine has no role in that if I remember correctly. Looked up the walkthrough, no Milking Machine:
Gwynn Demonologist Path (Ending Two) • Once Irith is cured, go to the Institute again • This time, you will observe two girls appearing and entering the Institute • Approach the pedestal and place the spider in there • Regarding the riddle, count the spiders and add seven, but eliminate the first digit (1->8, 9- >(1)6 etc…) • Ask for Gwynn to be enrolled • Ask Gwynn, she refuses • Hide the spider in her bedroom and wait for three nights • Talk to Gwynn again, and she agrees to her demonology training if you bring her a trainee collar • Go to the Institute, ask the Guardian to enter the dungeon and get the training collar • Send Gwynn three times to the Institute for lessons • When she returns from the third time, a cutscene with Nyx will play and the game will move to ending two NOTE: You can follow the ending two path until the second Institute lesson and still achieve the Goblin king ending Congratulations, you have achieved Ending Two
Hmmm, this issue has not been reported before. It might just be a gameplay issue.
I had a quick look into the code. SUB3 is not required for Gwynn to use the milking machine. Once you have built the machine, you can ask Gwynn to "test it out". In that cutscene she will get SUB 3 afterwards. So I assume you did not do that part with Gwynn yet.
If you play the game without mod you can send me a link to your a relevant save game file and I'll have a look.
I can't talk to Irith in the morning. She says "Have you found anything yet?" Grex says "I'm working on it" She then says "stop idling and go to the dungeons" but I cannot find anything in the dungeons to satisfy her. What am I missing?
I know I'm being a ball buster, but I'm stuck on my pet training. Using the leash to take her yo the brothel doesn't raise her sub anymore (it's at 20), but the quest to walk her is still there. Am I missing something?
The quest is not clearing (bug). Once you took her top the brothel and SUB is 20 its end of that content. That part is anyway optional for those who do not like brothel content.
About Nyx: In your walkthrough you wrote about increasing her SUB "Before sending Nyx to attend to Irith, go to the bathroom at 7:00 and switch the fragrance labels." Sorry, but I can´t find a way how to do it.

i can't figure out how to work the portal expansion minigame, nothing i press seems to do anything

The BUG Described in CR-0139 - I have the lower dungeon key but game still say i do not have it - huge game progress not knowing that :-(
Are you playing with mods like urm? Those will corrupt the game state, yielding weird quest lockups at times.
Maybe you forgot to report back to the mirror? That might also be a reason the quest does not switch?

I have find out, there is wrong name of the key in message, lower levels are acessible in late game with other key from the mage girl.

My whole problem is you cant even ask her to enroll until she has sub 3, which you get to with the milking machine

Theres a button "switch labels" in the bathroom at 7:00 if the quest is active
